A facelift isn't always the answer—which treatment is really right for you

Many people considering a facelift are primarily driven by a specific feeling: they look in the mirror in the morning and something no longer looks fresh—their eyes appear heavier, the crease in their eyelids is more noticeable, and the corners of their mouth droop slightly. This feeling is justified. But it doesn’t automatically mean that a full facelift is the right answer.

A classic facelift is a comprehensive procedure that addresses the cheeks, jawline, neck, and the deep structures of the face. It is the treatment of choice when the face as a whole has lost its firmness. Yet in many cases, the cause of the discomfort is more localized—and the solution, accordingly, smaller and more precise.

When one area defines the overall look

  • Eyelid lift (blepharoplasty): Heavy upper eyelids or bags under the eyes can make the face look significantly more tired.
  • Neck lift: If the neck—not the face—is the problem, an isolated neck lift can be very effective.
  • Wrinkle treatment with hyaluronic acid or Botox: If wrinkles without tissue sagging dominate the appearance, minimally invasive procedures are often more appropriate.
  • Thread lift: A minimally invasive alternative for mild sagging without surgical intervention.

Minilift – a gentle facelift for the first signs of aging

The mini-lift is particularly suitable for younger patients with mild wrinkles and early signs of excess skin in the cheek area. The procedure is performed through a small S-shaped incision in front of the ear. Only the skin is tightened—the muscles and deeper structures remain untouched.

Advantages:

  • Short recovery time
  • Can be performed on an outpatient basis under local anesthesia
  • Minimal scarring

Limitations: Because only the skin is tightened, the result is less permanent and may lead to a faster loss of elasticity. This method is not suitable for severely sagging tissue.

SMAS Facelift – the proven deep-layer lift

The SMAS facelift (superficial musculoaponeurotic system) is one of the most commonly used methods—and for good reason. It tightens not only the skin but also the deeper layers of connective tissue and muscle that are crucial for facial contouring.

Through precisely placed incisions at the hairline and around the ear, the skin and SMAS layer are separated and repositioned to create a more youthful appearance. Excess skin is removed, leaving the skin loosely draped over the tightened underlying structure—for a natural, relaxed result without a “mask effect.”

  • Sagging jawline (“jowls”)
  • Pronounced wrinkles on the neck

Benefits:

  • Long-lasting results

MACS Lift – the modern short-incision technique

The MACS facelift (Minimal Access Cranial Suspension) is a less invasive technique that involves smaller incisions, usually limited to the temple area. Here, too, the SMAS is tightened, but using vertical suspension threads that reposition the tissue deep within the tissue.

Ideal for:

  • Younger patients
  • Mild sagging in the midface
  • Desire for shorter recovery time

Advantages:

  • Fewer scars
  • Short surgery duration
  • Good compromise between a mini-lift and an SMAS lift

Limitations: The effect on the neck and chin area is limited—SMAS or deep-plane techniques are better suited for these areas.

Deep Plane & Superextended Facelift – The Comprehensive Solution

These techniques are particularly suitable for older female patients with significant tissue laxity. Unlike the SMAS facelift, the entire SMAS connective tissue and the neck muscle (platysma) are detached from the underlying tissue and reshaped as a single unit.

Advantages:

  • Comprehensive repositioning of all facial components
  • Extremely natural results, particularly in the cheeks, chin, and neck
  • Long-lasting results (often > 10 years)

Ideal for:

  • Advanced aging
  • Severe neck sagging

Technique:
